New Patient Registration
- To register at the Practice, please see the bottom of the page for the forms required. Please note that you must fill in an return the registration form AND the new patient health check along with photo ID and proof of address. If we don't receive all of these documents, fully completed your registration will be delayed as we need all documents to fully process your registration.
Culloden Surgery covers the Eastern side of Inverness up to the Military Road. We do not register new patients who reside on the western side of the A9 nor do we continue to register patients who move address to the other side of the A9. Patients living close to the Nairn & Ardersier Practices are outwith our boundary. We cover as far as Daviot to the south.
It would normally be possible to visit the surgery to request registration forms but due to the COVID pandemic patients are requested to fill in an online registration form (see bottom of page) which should be emailed to the surgery: firstname.lastname@example.org
Please title your email New Patient Registration.
Please ensure you have included an up to date contact telephone number with your application so that we can contact you should we need to. Please also complete a New Patient Questionnaire Form. It would be helpful if you could also provide one form of photo ID and evidence that you reside at a local address ( ie lease agreement, community charge bill).
The Surgery will acknowledge receipt of your email. If there is an issue with your application to register you will be contacted by a member of our team to discuss this with you.
Once you have completed the registration process your former medical records will be requested and transferred to the surgery.
If you move outside of the practice boundaries (including the western side of A9 or east of the Military Road towards Ardersier and Nairn), the surgery will ask you to register with a GP who is nearer to your home address.
