New Patient Registration

We welcome registrations from patients living within our practice boundary as outlined on the map below.  There are 2 practices within the building and we accept registrations on alternate weeks so please call the surgery to check which practice is registering before submitting any forms. 

Eligibility to use NHS services depends mainly on residence in the UK. You can only register with a GP Practice if you are staying in the area for more than three months. If you are not, you may still be treated, but as a Temporary Resident. For more information please visit NHS Inform.

Due to COVID, you should register online as we are limiting the number of people entering the building.  If you do not have access to a computer you can collect registration forms from our reception desk. 

Patients should fully complete a registration form and a new patient questionnaire (over 16 year olds only) and email these to  You should also email us proof of your current address and one form of photo ID along with your registration forms.

You should also read the patient information leaflet which gives information on our opening hours, the services we offer and how to order repeat prescriptions.

You will be registered with the Surgery and not with a specific doctor.  Once registered you can consult with a doctor of your choice.
